This program helps cover medically necessary clinic services for patients experiencing severe financial hardship, based on available funds and eligibility criteria. Sponsorship may be applied to qualifying patient balances according to policy and is limited and not guaranteed.

Evergreen Ember Wellness uses a tier-based pricing model to improve access while maintaining a sustainable, high-quality telehealth practice. Tier placement does not affect clinical decision-making or care quality.
Tier A and Tier B do not require documentation. Tier C and Tier D require a brief self-attestation, and availability may be capped to keep access open.
Care is telehealth only at launch, and patients must be physically located in Colorado during visits. Labs are coordinated through external facilities when clinically appropriate (Labcorp sites are preferred).
Limited sponsorship funds may be applied to visit fees, membership fees, and medically necessary labs ordered by the provider, when available.
