Evergreen Ember Wellness is a self-pay telehealth practice with sliding-scale visit tiers and membership options. If you are not sure which is best, explore both and decide with us during your first visit.

Membership Care vs Per-Visit Care
Member Care
What membership includes:
- Up to 25 scheduled telehealth visits, per calendar year (during active membership)
- Ongoing clinical assessment and medical decision-making over time
- Lab draws are included at no additional cost to the membership when completed through Evergreen Ember Wellness’ designated lab process.
- Portal messaging access for ongoing communication related to active care plans
- Medication management and refills as clinically appropriate
- Care coordination over time, including referrals, prescriptions, and clinically indicated orders
- Completion of forms, letters, disability paperwork, or legal documentation
Single or Dual Tracks of Care are available:
- Longitudinal gender-affirming care (GAC), including medication management and monitoring when clinically appropriate
- Inclusive LGBTQ+ Primary Care
- Or a combination of the above two listed tracks: GAC and LGBTQ+ Primary Care, including preventive care planning and chronic condition follow-up when appropriate via telehealth
Per-Visit Care (Non-Member)
Per-visit fees are designed for focused, episodic care.
Each per-visit fee includes:
- A scheduled telehealth visit with a licensed clinician
- Pre-visit review of submitted intake information, medications, and relevant prior labs (if available)
- Clinical assessment, medical decision-making, and shared decision-making appropriate to the visit
- Gender-affirming care services, including GAHT discussion, education, and medication management when clinically appropriate
- LGBTQ+ inclusive primary care support for telehealth-appropriate concerns
- Ordering of clinically necessary labs when indicated (performed at external facilities; Labcorp sites preferred when available)
- Care coordination related to the visit, such as sending prescriptions or placing lab/imaging orders
- Brief follow-up communications related to the visit (e.g., clarification of the care plan or normal results)
- Labs may be ordered and billed to insurance or billed to the clinic. Labs billed to the clinic, are available at reasonable self-pay costs. Because pricing varies by test, patients are encouraged to request our Common Lab Cost Sheet, which outlines many of the labs we commonly order and their estimated costs.
Sliding Scale Tiers
Evergreen Ember Wellness uses a tier-based pricing model to improve access while maintaining a sustainable practice. Patients select a pricing tier during intake. Tier A and Tier B do not require documentation. Tier C requires a brief self-attestation and may be capped.
Tier placement does not affect clinical decision-making, visit length, or quality of care. Pricing information is for general informational purposes.
Tier A: Standard rate for patients with more financial flexibility.

- Full clinical services and visit quality are unchanged.

Per Visit:
Gender Affirming Care (GAC) - New Patient $225 | Established Patient $150

Tier B: Mid-range reduced pricing without documentation.

Tier C: Lower-cost access tier with brief self-attestation.

Important notes
- All clinical decisions are made at the discretion of the provider.
- Telehealth-only at launch; patients must be located in Colorado during visits.
- Labs are coordinated through external facilities; Labcorp sites are preferred.

Exclusions and fees
- No-show fee: $50
- Late cancellation fee: $25 (less than 24 hours’ notice)
- Sliding scale pricing does not apply to third-party costs.
- Repeated no-shows may affect sliding scale eligibility.
